Post-Hospitalization Care in Massachusetts, Led by Nurses From Day One
When a loved one is discharged from the hospital, families often have days, not weeks, to plan. You need the right care in place before they come home.
Immaculate Homecare Services was founded by two nurses who know what the first week’s home can bring. Every post-hospitalization case we take in Massachusetts is managed by a nurse from day one, so the discharge plan is followed, new medications are tracked, and changes are caught early instead of becoming a trip back to the hospital.
If the discharge date is coming fast and you’re not sure who can safely help at home, you’re in the right place.

What Makes Our Post-Hospitalization Care Different From Other Agencies in MA?
Deciding a loved one needs care around the clock is one of the hardest calls a family makes. You need someone you can trust in the home, day and night.
The weeks after discharge carry real risk. About 16 of every 100 hospital stays for Medicare patients 65 and older lead to a readmission within 30 days, according to the Agency for Healthcare Research and Quality. Yet many agencies send a caregiver home with little clinical coordination.
Immaculate Homecare Services was built differently. Our owners are nurses, and a nurse care manager oversees every case from the first contact. That means the person managing your loved one’s recovery understands the discharge orders and can coordinate directly with the discharge planner, VNA and hospice teams, and physicians already involved.
We are not a franchise. We are not a referral service. We are a nurse-led private duty home care agency in Massachusetts, and we answer our phones.
Post-Hospitalization Care Services We Provide
Immaculate Homecare Services supports adults recovering at home after a hospital stay, from help with daily living to complex cases that require clinical oversight. Where families most often turn to us is in the harder transitions: the ones where a loved one comes home needing more than a standard caregiver can safely provide.
Our services include:
Unlike standard companion care agencies, Immaculate Homecare Services offers private duty nursing. This means a nurse can come to your loved one’s home multiple times per day if needed. Visiting nurses through the VNA typically come a few times per week after discharge. We can fill the clinical gap in between.

Frequently Asked Questions
How much does post-hospitalization care cost in Massachusetts?
Post-hospitalization care costs in Massachusetts typically depend on:
- The number of hours, overnight coverage, or 24/7 live-in care needed
- The level of acuity and any skilled nursing tasks involved
- Whether the agency has clinical oversight built into its model
- Whether the agency carries liability and workers’ compensation coverage for its caregivers
Lower-cost options often provide only basic supervision, without clinical oversight, skilled nursing tasks, hospice coordination, or the same insurance protections. Those costs can shift onto the family if something goes wrong during recovery.
At Immaculate Homecare Services, we don’t publish fixed rates because every recovery plan is different. Call us to talk through cost and what post-hospitalization care could look like for your family.
What is post-hospitalization care?
Post-hospitalization care is in-home support that helps an adult recover safely after a hospital stay. It can include personal care, medication support, skilled nursing tasks, and transportation to follow-up appointments.
How quickly can post-hospitalization care start?
In many cases, we may be able to start care the same day we discuss your needs. Our trained staff will conduct an assessment, including in-home, and we move quickly when a discharge date is close.
What is the difference between post-hospitalization care and home health care?
Home health care (typically VNA services) is medically focused, insurance-covered, and visit-limited, usually a few times per week. Post-hospitalization care through a private duty agency provides daily support during recovery, including overnights and 24/7 coverage, and with private duty nursing, can bridge the clinical gap between VNA visits.
When should families arrange home care after a hospital stay?
Ideally, before the discharge date. Arranging care early lets the home care team review discharge instructions and have support in place on the first day home.
Does Immaculate Homecare Services accept Medicare or Medicaid?
Immaculate Homecare Services is private pay only and does not accept Medicaid or Medicare. Some clients use long-term care insurance to offset costs. We’re happy to discuss how that works.
What areas does Immaculate Homecare Services serve?
We serve Greater Boston, Woburn, and surrounding communities across Middlesex and Merrimack Valley, Massachusetts.
Who will be managing my loved one’s care?
A nurse care manager oversees every case from the first contact. You’ll also receive updates and immediate notification of any changes to the care schedule.
