In-Home Hospice Care Support in Massachusetts

If your loved one is beginning hospice care at home, you’re probably realizing what most families discover quickly: the hospice team is not there around the clock, and the hours in between fall to you. You don’t have to carry those hours alone.

Immaculate Home Healthcare is a nurse-owned, nurse-led private duty in-home care agency based in Woburn, serving Greater Boston, Middlesex County, and Merrimack County. Both of our owners are nurses, and a nurse case manager is involved in every case from the very first phone call. We provide the continuous care and skilled nursing support that hospice alone does not cover – which is why hospice nurses and discharge planners across Greater Boston refer families to Immaculate more than any other agency.

In many cases, care can begin within 24 hours.

What Is the Difference Between Hospice Care and In-Home Hospice Support?

Hospice care is a Medicare-covered benefit that manages a person’s comfort at the end of life through scheduled, intermittent visits from a hospice nurse, aide, social worker, and chaplain. In-home hospice support is private duty care, both skilled nursing and hands-on caregiving, that covers the hours between those visits, up to and including 24-hour care at the bedside.

Most families are surprised to learn that hospice is not around-the-clock care. A hospice nurse may visit one to three times a week. A hospice aide may come for an hour a day to help with bathing. The rest of the time falls to the family.

Immaculate Home Care can fill those gaps in time for you with the clinical skill of private duty nursing.

How Does In-Home Hospice Care Work in Massachusetts?

In Massachusetts, hospice care is delivered through what Medicare calls the hospice benefit, a program of intermittent nursing visits, aide support, and comfort-focused medical care for patients near the end of life. The hospice benefit is covered by Medicare at little to no cost to the family.

Families who want continuous care at home add private duty nursing services to cover the hours the hospice benefit does not.

The Immaculate Homecare Difference

When private duty nursing is added, the hospice care team and the private duty nursing agency coordinate together so the plan of care stays consistent. This is where Immaculate is different: because our agency is owned and led by nurses, we speak the same clinical language as your hospice team from the very first phone call. There is no waiting for a nurse to be assigned or a clinical review to be scheduled – the person taking your call understands the diagnosis, the urgency, and what the care plan requires. That is why in many instances we can provide care the same day, and why the transition home feels seamless instead of chaotic.

How That Benefits Your Loved One

When a symptom changes at 2 a.m., a companion caregiver from a franchise agency can only call the hospice line and wait. An Immaculate nurse at the bedside can assess what’s happening, respond within her scope, and give the hospice team a clinical report, not a guess.

Why Do Families Add Private Duty Care During Hospice?

Families typically add private duty support during hospice for three reasons: the physical demands of round-the-clock caregiving exceed what one family can sustain, overnight hours become unmanageable, or family members live too far away to be present every day.

Our clients’ families tell us the same thing again and again: the private duty caregiver is what allowed them to stop being the nurse and go back to being the daughter, the son, the spouse. That time matters. It is often the time families remember most.

What Does Immaculate’s Hospice Support Include?

Our end-of-life and hospice support care includes:

  • Private duty nursing: medication management, comfort and symptom care, wound care, and clinical monitoring between hospice visits
  • Personal care: bathing, grooming, toileting, and dressing with dignity
  • Repositioning and comfort measures
  • Overnight and 24/7 live-in care options
  • Meal preparation and hydration support
  • Companionship and a calm presence for both the patient and the family
  • Respite for family caregivers who need rest to keep going
  • Regular family check-in calls and immediate notification of any schedule changes

Every case is overseen by a nurse case manager from the moment you call. Caregivers are matched to end-of-life cases based on experience, and nurses are supervised by owners who are nurses themselves.

How Quickly Can Care Start?

Care can often begin the same day you call. End-of-life situations do not wait for a five-day intake process, and neither do we.

This is one of the reasons hospice nurses and hospital discharge planners in Greater Boston refer Immaculate Homecare more than any other senior home care agency. If your loved one is being discharged home on hospice this afternoon, give us a call.

If your family needs support today, call 781-281-8078 now. We can often have a caregiver in the home within hours.

Frequently Asked Questions About Hospice Care

Does hospice provide 24-hour care at home?

No. Standard hospice care includes intermittent visits from nurses, aides, and other team members, typically a few hours per week total. Continuous care at home is usually arranged separately through family caregivers or an in-home care agency offering private duty nursing. Hospice does offer short-term continuous care during acute symptom crises, but this is temporary and medically determined.

Who pays for hospice care in Massachusetts?

Hospice care itself is covered by Medicare, Medicaid (MassHealth), and most private insurance for patients who meet eligibility criteria. Private duty nursing and caregiving that supplement hospice are generally paid out of pocket or through long-term care insurance, as they fall outside the hospice benefit.

Can someone be on hospice and still have a private caregiver or private duty nurse?

Yes. Hospice and private duty home care commonly work together. The hospice team manages the overall plan of care while private duty nurses and caregivers provide continuous care, clinical monitoring, and support during the hours hospice staff are not in the home.

How do you know when it's time for hospice?

Hospice is generally appropriate when a physician certifies that a patient has a life expectancy of six months or less if the illness follows its expected course, and the goals of care shift from curing the illness to comfort and quality of life. The patient’s doctor or a hospice agency can perform an evaluation to determine eligibility.

What is the difference between palliative care and hospice care?

Palliative care focuses on comfort and symptom relief at any stage of a serious illness and can be provided alongside curative treatment. Hospice care is a specific form of palliative care for patients near the end of life who are no longer pursuing curative treatment.

Can hospice care be provided at home in Massachusetts?

Yes. Most hospice care in Massachusetts is delivered in the patient’s home, whether that’s a private residence, an assisted living community, or a nursing facility. Hospice team members travel to the patient rather than the patient relocating to a facility.
